Position Overview
The Functional Medicine Nurse Practitioner will play a key role in evaluating, diagnosing, and managing patients through a personalized, integrative lens. You’ll work collaboratively within our dynamic team, providing comprehensive, whole-person care that restores health, vitality, and balance.

Key Responsibilities
   •   Conduct in-depth patient consultations, including history, nutrition, lifestyle, and environmental assessments
   •   Develop customized treatment plans focused on the root cause of dysfunction rather than symptom management
   •   Order, interpret, and explain advanced functional medicine testing (GI-MAP, DUTCH, OAT, hormone panels, nutrient analysis, and toxin screens)
   •   Provide evidence-based recommendations on nutrition, supplementation, peptide therapy, and bioidentical hormone replacement therapy (BHRT)
   •   Monitor and track patient progress through follow-ups and clinical re-evaluations
   •   Collaborate with our aesthetics, IV therapy, and wellness teams to support full-body rejuvenation
   •   Document accurately and comprehensively in EMR, ensuring continuity of care
   •   Stay informed of emerging research, protocols, and continuing education in functional and integrative medicine
   •   Empower patients through education, motivation, and lifestyle transformation



Qualifications
   •   Licensed Nurse Practitioner (NP) in the state of Minnesota
   •   Background in Primary Care, Family Medicine, or Internal Medicine
   •   Demonstrated interest or certification in Functional Medicine, Integrative Medicine, or Anti-Aging Medicine (IFM, A4M, or equivalent preferred)
   •   Experience or strong interest in hormone optimization, longevity medicine, weight loss, and metabolic health
   •   Exceptional communication and interpersonal skills
   •   Empathetic and patient-centered approach to care
   •   Self-starter with the ability to work independently while collaborating with a multidisciplinary team


Preferred Skills & Experience
   •   Knowledge of nutraceuticals, lifestyle medicine, and peptide protocols
   •   Familiarity with functional testing and root cause frameworks
   •   Experience using EMR systems and developing care pathways
   •   Desire to grow within a rapidly expanding functional medicine clinic

Primary Care / Urgent Care Physician or Nurse Practitioner

We are expanding our footprint and opening a new Forest Lake location, and we’re seeking a dedicated Physician (MD/DO) or Nurse Practitioner (NP) to join our growing clinical team. Position Overview
The Primary Care / Urgent Care Provider will deliver comprehensive, evidence-based care to patients of all ages. This role blends traditional medicine with a wellness-oriented, patient-first approach. You will manage both acute and chronic conditions, provide preventive care, and help patients achieve optimal health through education, diagnostics, and individualized treatment.

Key Responsibilities
   •   Provide comprehensive primary care and urgent care services, including wellness exams, chronic disease management, and acute visits
   •   Diagnose and treat a wide range of conditions (respiratory infections, lacerations, strains, infections, and other urgent needs)
   •   Perform in-office procedures such as suturing, splinting, injections, and I&D
   •   Order and interpret labs, imaging, and diagnostic tests
   •   Create individualized, evidence-based treatment and follow-up plans
   •   Educate patients on health maintenance, prevention, and lifestyle optimization
   •   Collaborate with our functional medicine and aesthetics teams to support integrated care
   •   Maintain accurate and timely EMR documentation
   •   Contribute to a positive, team-based clinic culture that reflects our mission and values

Qualifications
   •   Licensed Physician (MD or Nurse Practitioner (NP) in Minnesota
   •   Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Urgent Care
   •   2+ years of experience in primary or urgent care preferred
   •   Strong clinical judgment, procedural proficiency, and diagnostic skills
   •   Excellent communication and interpersonal skills
   •   Passion for patient-centered, compassionate, and efficient care


Preferred Experience
   •   Interest in preventive medicine, wellness, or functional medicine
   •   Experience in hormone optimization, weight management, or integrative care
   •   Familiarity with EMR systems and telemedicine workflows

Mental Health Nurse Practitioner or Counselor / Therapist

We are expanding our mental wellness services and seeking a Mental Health Nurse Practitioner (PMHNP) or Licensed Therapist / Counselor (LPCC, LICSW, LMFT) to join our growing team at our Forest Lake location. This role is ideal for a provider who values connection, holistic care, and the freedom to practice in a supportive, patient-centered environment.

Position Overview

The Mental Health Provider will deliver compassionate, evidence-based mental health care as part of an integrative clinical team. This role blends traditional psychiatric and counseling approaches with lifestyle, nutrition, and functional wellness principles. You’ll have the opportunity to shape our mental wellness program while working collaboratively with medical, aesthetic, and functional medicine providers who believe in treating the whole person.



Key Responsibilities
   •   Conduct comprehensive mental health evaluations and diagnostic assessments
   •   Develop individualized treatment plans tailored to each patient’s goals and lifestyle
   •   Provide ongoing therapy, medication management (for PMHNPs), and mental health support
   •   Collaborate with other clinic providers to integrate medical, hormonal, and emotional care
   •   Educate patients on stress management, sleep hygiene, nutrition, and lifestyle strategies that support mental health
   •   Maintain thorough, accurate, and confidential patient documentation
   •   Stay current on emerging therapies and evidence-based practices in psychiatry and counseling
   •   Help develop innovative mental wellness offerings within the clinic


Qualifications

For PMHNPs:
   •   Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) in Minnesota
   •   Experience in medication management, diagnosis, and integrative care preferred
   •   Strong communication, empathy, and clinical judgment

For Counselors / Therapists:
   •   Licensed LPCC, LICSW, LMFT, or related license in Minnesota
   •   Background in trauma-informed care, CBT, DBT, or integrative therapy models
   •   Passion for holistic mental wellness and collaborative treatment planning

General Requirements:
   •   2+ years of clinical experience preferred
   •   Comfort with both independent and team-based care
   •   Excellent communication and organizational skills
   •   Commitment to compassionate, patient-centered practice


Preferred Experience
   •   Familiarity with integrative or functional medicine principles
   •   Interest in hormonal, nutritional, or lifestyle impacts on mental health
   •   Experience supporting patients through anxiety, depression, stress, and life transitions
   •   Background in collaborative care, mind-body medicine, or wellness program development
